| Tell health care provider injury occurred at work |
The health care provider (HCP) needs to know if you are treating for a workers' compensation injury. Be specific about how and when the injury occurred, your symptoms and why you feel the injury is work-related. Provide as much of the following information as possible: the name of your employer; the name, phone number and fax number of your supervisor; and the name, phone number and fax number of your employer's insurance carrier.
