| Suggestions for a successful doctor visit | |
| 1. | Bring the following to every doctor visit:

| 2. | Be on time to all appointments; be sure to give plenty of notice to your doctor's office if you must cancel. |
| 3. | Give a detailed and accurate description of how your injury occurred and any body part that was affected. |
| 4. | Be thorough with your doctor. Explain if your symptoms are improving, worsening or staying the same. |
| 5. | Be prepared to give a complete and accurate medical history. |
| 6. | Ask how long it will take to recover from your injury and how long it will take for you to return to work, with and without restrictions. Ask for a copy of a Report of Work Ability. |
| 7. | Ask about less invasive or other treatment options. |
| 8. | If your doctor gives you prescriptions, referrals or restrictions, ask the doctor's office to fax all of the information to your adjuster. |
| 9. | Follow the treatment plan prescribed by your doctor. |
| 10. | Keep copies of everything for your own records. |
| 11. | Stay in open communication with your adjuster regarding all of your doctor appointments. |
