Update an Existing License
Business and contractor licenses often require the designation of a person responsible for the business' or contractors' compliance with the licensing requirements. Businesses and contractors that change their responsible person must notify the department of the change and submit the required documentation to designate a new person.
Electrical Contractor -- The new responsible Class A Master Electrician must complete and submit the Certificate of Responsible Licensed Individual Master Electrician form (PDF) before the designation will take effect. The designation must be submitted within 30 days after the previous designated responsible person left employment or ceased being the responsible electrician.
Elevator Contractor -- The new responsible Master Elevator Constructor must complete and submit the Certificate of Responsible Licensed Individual Master Electrician/Elevator form (PDF) before the designation will take effect. The designation must be submitted within 30 days after the previous designated responsible person left employment or ceased being the responsible electrician.
High-Pressure-Piping Business -- The licensed High-Pressure-Piping Business must notify DLI in writing of a change in its license Contracting Pipefitter within 30 days after the previous designated responsible contracting pipefitter left employment or ceased being the responsible contracting pipefitter. The notice to DLI must include the HPP Business' license number, name and address and the licensed contracting pipefitter's legal name and license number. No form currently exists to facilitate this change.
Manufactured Home Installer -- A licensed installer that changes its Qualifying Person must submit an Application for Change of Qualifying Person (PDF) before the change in qualifying person will take effect. A new qualifying person will need to provide the original exam passing score reports to document the person has passed the appropriate license exam (test results expire two years after the test was taken). Exam bulletins and registration are available online at www.pearsonvue.com or by phone at (877) 258-9221. The licensed installer has 120 days from the date the previous qualifying person ceased being the qualifying person to designate a new qualifying person.
Plumbing Contractor - A plumbing contractor that changes the designated responsible Master Plumber, Restricted Master Plumber or Certified Pipe Layer must submit an Application to Change Responsible Licensed or Certified Person (PDF) before the change will be reflected on the filing of the Plumbing Contractor Code Compliance Bond.
Residential Building Contractor -- A licensed residential building contractor that changes its Qualifying Person must submit an Application for Change of Qualifying Person (PDF) before the change in qualifying person will take effect. A new qualifying person will need to provide the original exam passing score reports to document the person has passed the appropriate license exam (test results expire two years after the test was taken). Technology System Contractor -- The new responsible Power Limited Technician must complete and submit the Certificate of Responsible Licensed Individual Power Limited Technician form (PDF) before the designation will take effect. The designation must be submitted within 30 days after the previous designated responsible person left employment or ceased being the responsible power limited technician.
Water Conditioning Contractor -- A bonded water conditioning contractor who changes the designated responsible licensed water conditioning contractor must submit an Application to Change Responsible Licensed Person (PDF) before the change will be reflected on the filing of the Water Conditioning Contractor Bond.
