Certificate of employment and supervisor forms: These forms, which are not available online, are a set of four forms for registration of an employer's responsible master electrician, maintenance electrician, electrical engineer or power-limited technician for electrical work performed on the employer's premises. To attain these forms, email your request to DLI.License@state.mn.us.
Please Note: The Department of Labor and Industry does not accept cash as payment for licenses, inspections, copy requests and other DLI services. Accepted methods of payment include checks, money orders and electronic payments that can be made through the DLI Web site.
